# Forevue Insights — Applicant Informed Consent

## Notice to Applicant

Read this document in full before proceeding. Checking the acknowledgment box at the bottom constitutes your informed consent to the data collection, processing, and use described below. You may not proceed with the application if you do not consent.

## 1. Who Is Collecting This Information

Forevue Insights ("Forevue") is a pre-employment screening platform contracted by the law enforcement agency to which you are applying ("Agency"). Forevue operates as a data processor on behalf of the Agency. The Agency is the data controller and employer of record for all hiring decisions.

## 2. What You Are Being Asked to Complete

This platform administers two instruments as part of the Agency's pre-offer applicant screening process:

**Biographical Questionnaire.** A structured questionnaire covering employment history, personal conduct, financial history, and other background information consistent with California POST 2-251 Personal History Statement requirements and any supplemental questions specified by the Agency.

**Occupational Behavioral Tendency Profile.** A 120-item self-report inventory measuring normal-range personality traits relevant to occupational performance in law enforcement contexts. This instrument is the IPIP-NEO-120, a public domain instrument developed by John A. Johnson, Ph.D. (2014). It measures five broad trait domains — Conscientiousness, Emotional Stability, Extraversion, Agreeableness, and Openness — across 30 facets.

## 3. What These Instruments Are Not

- Neither instrument is a psychological examination, clinical assessment, or medical evaluation.
- Neither instrument diagnoses, screens for, or identifies any mental health condition, disability, or disorder.
- Neither instrument produces a suitability determination or a recommendation for or against hire.
- Completion of these instruments does not substitute for the post-offer psychological evaluation required under California POST Regulation 1955, which occurs separately and only after a conditional offer of employment.
- These instruments are administered pre-offer and are designed exclusively to measure normal-range behavioral tendencies relevant to law enforcement occupational performance.

## 4. How Your Responses Are Used

**By the Agency.** Your completed screening profile is transmitted to the Agency's authorized personnel for use in the pre-offer applicant review process. The Agency makes all hiring decisions. Forevue does not make, recommend, or influence hiring decisions.

**By Forevue — Platform Operations.** Forevue retains your responses and scored profile to generate reports, maintain application records, and support platform security and integrity monitoring.

**By Forevue — Research and Norm Development.** With your consent, Forevue may use your de-identified responses as part of an aggregate dataset for the following purposes:

- Development and validation of law enforcement applicant personality norms specific to California POST agencies.
- Analysis of the predictive relationships between pre-offer screening data and post-hire outcomes, using de-identified outcome data provided by agencies.
- Research publication using pooled, de-identified data in academic or professional venues. No individual applicant will be identifiable in any published output.
- Re-scoring of raw item responses against future Forevue-developed norm sets. Raw responses are retained to permit re-analysis as the normative reference group expands. Re-scored results are used for research purposes only and do not alter or replace the scored profile transmitted to the Agency at the time of your application.

**De-identification standard.** Before any data is used for research or norm development, direct identifiers (name, date of birth, social security number, address) are removed and replaced with a one-way cryptographic hash. The hash cannot be reversed to recover your identity. Your responses are not sold, licensed, or shared with any third party outside the purposes described in this notice.

## 5. Data Retention

Your application record, including biographical questionnaire responses and personality profile scores, is retained for a minimum period consistent with applicable California recordkeeping requirements for law enforcement pre-employment screening. Raw item-level responses are retained indefinitely for research and re-scoring purposes in de-identified form. You may request information about your record by contacting the Agency's human resources office.

## 6. Your Rights

You have the right to:

- Decline to consent. If you do not consent, you may not proceed with this platform. Contact the Agency's human resources office to discuss alternative application procedures, if any are available.
- Request access to your application record through the Agency.
- Request correction of factual errors in your biographical questionnaire responses through the Agency prior to submission.

Once submitted, biographical questionnaire responses are locked and may not be edited. The occupational behavioral tendency profile is administered in a single session and is locked upon completion.

## 7. No Employment Guarantee

Participation in this screening process does not guarantee an offer of employment, advancement in the hiring process, or any other employment outcome. The Agency retains full discretion over all hiring decisions.
